Children passports eligible to be mailed in (form PC8)

HOW TO RENEW AN AUSTRALIAN PASSPORT FOR CHILDREN UP TO AND INCLUDING AGE 15 BY MAIL/POST (PC8 form)

You can use this form and apply by MAILING IN the application to the Embassy if your child is under 16 and has already had a passport.

If you don't meet this criteria, the application form will need to be submitted in person and by appointment only.

Please use the check list below before you send your application to the Embassy.

Child PC8 Passport Application CHECK LIST

1. Did you complete the PC8 passport application form (in black pen) that you either printed from the Australian Passport Office website or received from the Australian Embassy? 

 

2. Is the current home address, contact phone number and email address on the form valid and correct? 

 

3. Are the biodata personal details of the child correct on the application form? (Name, Place of Birth, Country of Birth, Date of Birth and Sex) 

 

4. Did you include a minimum of two passport photos that are not older than six months? Please do not stick the photos in the form.

 

5. *Do the photos meet the photo guidelines?

 

6. Did the guarantor endorse the back of one photo with a black pen? 

 

7. Are you sending photocopies of both parents proof of identity showing name, photo, signature and current address? These can be Croatian or Australian ID documents.

 

8. **Are both parents’ names the same as how they appear on the child’s birth certificate?

 

9. Did the guarantor complete and sign section 11?

 

10. Did both parents complete and sign section 15 (declaration and consent) and an authorised person witnessed their signatures? 

 

11. Did one parent (or person with parental responsibility for the applicant) sign Section 17?

 

12. If the child is aged 10 and over, did they sign Section 18? Make sure the child’s signature does not go outside of the white box.  

 

13. Check all signatures are within six months of mailing the application.

 

14. Check all signatures are within the white signature box without any markings outside the box!

 

15. Check the form is printed in full and no parts of the form are cut short?

 

16. Have a bank card ready to make payment when you are contacted by the Embassy.

Please note that all passport applications lodged overseas will incur an additional fee (overseas surcharge). The overseas surcharge will not apply to emergency passport applications.

We accept only electronic payments or payment by bank card at the counter (Mastercard, Maestro, Visa, Visa Electron, VPay).  We do not accept cash payment.

** If the current names of either parent are different from those on the birth certificate, please include certified copies of documents that explain the change of name.

  • - The copies are certified by an authorised person by writing the words: “THIS IS A TRUE COPY OF THE DOCUMENT PRESENTED TO ME”
  • - The authorised person then signs, dates and stamps with their occupation stamp under these words.
  • - The following are authorised person occupations: doctor, lawyer, notary public, dentist, veterinarian, public servant.
